Use of this form is limited to parking ticket appeals for vehicles owned and operated by employees of Kutztown University and its affiliates. It is not to be used by students or visitors.

This form is limited to business-related Enterprise vehicle rentals. Rentals for personal use should NOT be submitted via this form. Employees may request a promotion number from KU Business Services to enter when reserving a vehicle for personal use via a home computer or mobile device.

Complete in order to apply for a special parking permit. Please have medical documentation available to attach to document.

Complete this form to rent 1-3 campus shuttle buses for use on campus and within the immediate area of KU. Shuttles are available for rent at the following rates and only outside normal campus shuttle operating hours and with the approval of the shuttle service provider.
