When a student is interested in pursuing an internship/field experience for academic credit, they must meet with their faculty advisor to discuss and confirm that the advisor will advise/monitor the student during the internship. Once confirmed, the Department Secretary submits the form, which captures student contact information, the proposed semester for internship registration, course information, and necessary e-signatures. The form is signed by the faculty advisor, then the department chairperson, who verifies student eligibility, and then the dean of the college. The director of clinical experiences must also sign the form for students in the College of Education.

The Intern Certification Application, Internship/Field Experience Registration and the Site Approval Form are essential documents that must be completed before beginning any approved internship placement.
